With the marquee tournament just four weeks away, the southpaw is in a race against time to regain full fitness, putting his participation in the competition in doubt.
Tilak was rushed to a hospital in Rajkot on the morning of January 8 after experiencing severe abdominal pain shortly after breakfast. The incident occurred while he was representing Hyderabad in the Vijay Hazare Trophy 2025-26. Medical examinations revealed testicular torsion, a condition requiring immediate surgical intervention. According to reports, doctors have advised a recovery period of two to four weeks, making his availability for the upcoming T20 World Cup uncertain. As a result, the southpaw has reportedly been ruled out of the five-match T20I series against New Zealand, scheduled to begin on January 21.

Sources close to the development stated that Tilak felt intense pain on January 7 and was promptly taken to the hospital, where scans were conducted and shared with doctors at the BCCI’s Centre of Excellence. Following consultations, surgery was recommended, and the procedure was successfully carried out. However, medical experts believe the recovery timeline leaves little margin for error, with the T20 World Cup starting in early February.
The yongster was one of India’s best batters in the Asia Cup 2025, playing a match-winning role in the final. Meanwhile, in 37 innings, he has scored 1,183 runs at an average of 49.29 and a strike rate of 144.09, including two centuries and six half-centuries. Notably, all his T20I fifties have come while chasing, with an average of 68 in run chases.
With Tilak being ruled out of the T20I series against New Zealand, the team management is expected to name a replacement soon. While Suryakumar Yadav is the designated No. 3 batter, Tilak’s absence could force India to reshuffle their middle order once again.

Testicular torsion is a medical emergency in which the spermatic cord twists, cutting off blood supply to the testicle and causing sudden, severe pain in the lower abdomen or groin. Tilak, who initially presented with acute abdominal pain, led to immediate hospitalisation and scans. Doctors identified testicular torsion and advised urgent surgery, which he has since undergone. Such procedures typically require a recovery period of two to four weeks, depending on the severity and response to treatment.
